Accessing DS Forms as a Bureau Coordinator

As a Bureau Coordinator, accessing the DS forms must be done through REA Management. It is important to understand that in order for the forms to populate any hours, the REA or the Coordinator will need to ensure that the total hours worked for that time period are listed in the Time Sheet Entry section of the REA Home page.

 

To navigate to the REA Home page, click REA Management from the left tool bar and in the dropdown menu, click REA Home. Next, type the REA you are looking for in the search bar at the top of the page and click the REA's name when located. This displays the REA Home page for that REA.

 

First, ensure that the total hours worked by the REA during that time period have been entered into the Time Sheet Entry section of the REA's Home page. When the total hours for the time period have been entered, the time sheet will then need to be submitted by clicking the Submit Timesheet button below the Time Sheet Entry section.

 

 

Once the time sheet has been submitted, a View DS Form button displays in place of the Submit Timesheet button. Click the View DS Form button and there may be up to 3 different DS Form options.

 

 

The first option is the DS-4151 form that displays the official time sheet for the hours worked during that time period for the REA.

 

 

The next option is the DS-3060 form that displays any overtime hours worked by the REA during that time period for the REA.

 

 

Lastly, if the REA has been transferred to another bureau outside of their primary bureau to fulfill an assignment, there is a DS-7603 form. This is the Transfer of Labor form that displays the information and hours related to the time worked at the transferred bureau.

 

 

Each of the forms allows the options to download, print, and email the form(s) for further use.
